Patients can now cancel their appointment with the Student Health & Wellness Centre online using this form. Once you have submitted your cancellation request, you will receive an autoreply to show that your cancellation has been submitted and will be processed. This form is not to be used for appointments at the Halifax Student Health & Wellness Centre only [you must contact Dal Family Medicine or the Student Health and Wellness Centre in Truro to cancel appointments at those clinics].

Please note: Student Health & Wellness has a cancellation/no show policy for medical appointments that requires 24 hours' notice of cancellation or you will be charged a missed appointment fee.
