Senator question form
The Senator Question Form is used by Senators to seek information on matters within the jurisdiction of the Senate that arise outside of Senate meetings. Before completing the form, review the Senator Question Protocol.
Submit your completed form to senate@dal.ca at least 10 calendar days prior to the Senate meeting.
Notice of senate academic appeal form
The Senate Appeals Committee hears student academic appeals from faculty-level decisions. If you are a student who wants to appeal an academic decision made by your faculty:
Review the Academic appeals information on the Senate Appeals Committee page, including the SAC Jurisdiction and Appeals Procedures to ensure you meet the appeal criteria.
If you meet the criteria and would like to proceed with an appeal, complete the Notice of Academic Appeal Form and email it to discipline.appeals@dal.ca.
Notice of senate discipline appeal form
The Senate Appeals Committee hears student discipline appeals from Senate Discipline Committee decisions. If you are a student who wants to appeal a Senate Discipline Committee decision:
Review the Discipline appeals information on the Senate Appeals Committee page including the SAC Jurisdiction and Appeals Procedures to ensure you meet the appeal criteria.
If you meet the criteria and would like to proceed with an appeal, complete the Notice of Discipline Appeal Form and email it to discipline.appeals@dal.ca.
Honorary degree nomination form
Through the awarding of honorary degrees, Dalhousie aims to recognize individuals whose accomplishments and achievements will inspire our graduates while reflecting the character and values of the university. The Senate Honorary Degree Committee (SHDC) (link to SHDC page) invites nominations of candidates who have demonstrated:
i. Inspirational leadership in, or service to, society;
ii. Outstanding contributions to, or leadership in, a field or discipline of study or a non‐academic area of achievement;
iii. Significant pattern of outstanding achievement at, or contributions to, Dalhousie University.

If you are ready to proceed with a nomination:
Complete the Honorary Degree Nomination Form
Attach the following supporting documents:
1. A curriculum vitae or career summary of the nominee (e.g., an excerpt from Who’s Who).
2. A personal statement of support for the nominee.
3. A summary of the nomination.
4. At least two letters seconding/supporting the nomination.
Submit your completed form and supplementary documents by email to senate@dal.ca.
