I completed these requirements as a part of my BScN at Dalhousie. Do I still have to submit them? Can’t you just access them? | For multiple reasons, including privacy regulations and ensuring that documentation is current and active, the short answer is no. All students entering the MN NP Program must submit the required pre-placement documentation. You will be provided with a link, which will allow you to upload and provide dates of this documentation. |
Who may sign them for me? | Faculty of Health Immunization and TB forms that have been provided to you from the School of Nursing are to be completed by a health provider or Occupational Health Nurse. |
I had the two-step TST in the past. Do I have to get it again? | Please follow the instructions on the form. If you have had a 2-step TST, a 1-step test is required. Please list dates of 2 step TST and date of 1 step TST, signed by a health provider or Occupational Health Nurse. |
Can I just send you my Nova Scotia Health Occupational Health Record? | We can accept only the Faculty of Health Immunization and TB forms that have been provided to you from the School of Nursing, completed by a health provider or Occupational Health Nurse. |
