Benefits package for grant-paid employees
[full-time and part-time (50% or greater) employees for contracts of at least 8 months or greater]
Forms for new employees
- Mandatory forms
- Health/Travel and Dental Application
- Late Medical Application (complete if applying 60 days after hire date)
- Life, AD&D Beneficiary
- Registered Retirement Savings Plan (RRSP)
- Grant holders must provide eligible Grant-Paid employees the option to contribute to the RRSP. If opting into the RRSP, the grant holder must match the employee's selected contribution rate of 3%, 4%, or 5%. If enrolling in the RRSP, there is a two-step process.
- Step 1: Complete the RRSP Enrollment Form (login required)
- Step 2: Complete the Manulife RRSP Enrollment

- This form is required if an employee wishes to enroll in optional benefits.
- Long-Term Disability
- Evidence of Insurability is required if an employees wishes to apply for long-term disability 31 days after their hire date.

- Voluntary AD&D
- Critical Illness & Assured Access (direct enrolment through Blue Cross)
- Benefits Authorization Form
